Officers responded to call from Richard McClendon's mother who said he was having a violent episode, destroying things inside the home and possibly had a shotgun. Officers arrived to McClendon inside the bathroom and they tried to get him to come out. McClendon then confronted the officers with a knife in one hand and a pipe in the other. One of the officers tried to subdue McClendon by tasing him. When that failed, they opened fire on him once, fatally wounding him
